Resources for community services
Community Services are valuable allies in reaching many people living in your community – often those who can benefit most from information and support for making healthy choices.
Community services includes many settings that may often be overlooked – they run programs that reach new families, newcomers to Canada, and families who face health challenges, or may also be faith-based or cultural organizations. They are an integral part in providing information and access to healthy choices to children and their families.
We have included information and resources here for the Early Years population (children aged 0-4 years) since childcare providers are also important providers of support, information, and healthy choices to young children and their parents and caregivers. While the Live 5-2-1-0 recommendations are evidence-based for the 5-12 years age group, we’ve contextualized the message for the Early Years because developing healthy habits early on sets children on the path to a healthy weight as they grow and develop.
SCOPE and our partner communities have developed a number of resources and ideas that might be useful for your Community Services programs – learn more below.
Through its partnership with SCOPE, Healthy Abbotsford has developed a Live 5-2-1-0 Activity & Resource Toolkit which provides activities, materials and information that service providers can use to share and support the Live 5-2-1-0 message. The Toolkit would be useful for providers such as daycare workers, early childhood educators, teachers, coaches, community health workers, and even parents. We’ve adapted Abbotsford’s version so that this Toolkit can be easily used in other communities too!
The Live 5-2-1-0 Activity & Resources Toolkit has sections specific to children age 0-4 years old and age 5-12 years old. For 5-12 year olds, the Toolkit has eleven different Live 5-2-1-0 activity ideas (including instructions, and in some cases materials), as well as reading suggestions for books available in local libraries. The Toolkit also provides information for individuals, organizations, childcare providers and schools on further steps that can be taken to share and support Live 5-2-1-0.
The Live 5-2-1-0 Activity & Resource Toolkit can be downloaded as a PDF file and used right away, or communities can download the adaptable Adobe InDesign version and add their own community logos and local program information. Through its partnership with SCOPE and Fraser Health, Healthy Abbotsford developed a script that was originally used by Public Health Nurses to introduce the Live 5-2-1-0 message to new families (more information on this initiative can be found in the ‘Health’ Sector). This Live 5-2-1-0 script is also useful for other Community Service Providers, who can use it to introduce and discuss the Live 5-2-1-0 message with children and families in a number of settings.

The Chilliwack Division of Family Practice, in partnership with SCOPE, has created a fantastic and very extensive toolkit to support service providers in teaching kids and families about healthy living using the Live 5-2-1-0 message. This Toolkit provides information on the 5-2-1-0 guidelines, healthy living ideas, ‘how-to’ instructional resources, easy recipes and fun game templates to help promote the message.
The Live 5-2-1-0 Service Providers Toolkit is available for download below. If you are interested in creating an adapted version of the Toolkit specific to your community, including a list of local programs and services for children and families, contact SCOPE.
The Live 5-2-1-0 Weekly Goal Tracker Sheet can be used to help kids and families set goals for daily healthy behaviours relating to the elements of the 5-2-1-0 message, and track their daily progress towards achieving their goals. Goals can be personalized, and ideas and tips are suggested to help make healthy choices easier.

The Live 5-2-1-0 Colouring Sheet for kids is available for download below – kids can learn the Live 5-2-1-0 message while colouring in the logo and active, fun images. This black & white file is easy to print on office printers!
The Live 5-2-1-0 Fact Sheet: Setting the Stage for Infants, Toddlers, and Preschoolers (0-4 year olds) was developed from Healthy Abbotsford’s Activity & Resource Toolkit.
While “Live 5-2-1-0” is an appropriate goal for children aged 5–12, the guideline needs to be adjusted slightly for children aged 0–4. This fact sheet contains a modified version of the Live 5-2-1-0 guideline for children aged 0-4.
The fact sheet is available for download below, and can be adapted to add your community organization’s logo! It also comes in seven translated versions – Arabic, Chinese, Korean, Spanish, Vietnamese, Punjabi and Filipino (Tagalog), through the invaluable support of Fraser Health who provided the translations.
In partnership with a team at Fraser Health, four information sheets were developed to address physical activity and nutrition during the early years (0-4). Four resources are available:
1. Live 5-2-1-0 Early Years Nutrition Sheet (birth to 12 months)
2. Live 5-2-1-0 Early Years Nutrition Sheet (1 to 4 years)
3. Live 5-2-1-0 Early Years Physical Activity Sheet (birth to 12 months)
4. Live 5-2-1-0 Early Years Physical Activity Sheet (1 to 4 years)
The two Nutrition sheets have also integrated Ellyn Satter’s Division of Responsibility in feeding messaging. They are all available for download below, and can be adapted to add your community organization’s logo!

The Live 5-2-1-0 Screen Time Fact Sheet provides background for the guideline of no more than two hours of screen time a day, including its implications for children’s health and well-being.
You can download and print the Screen Time Fact Sheet below. Share it with parents, caregivers, and other service providers to promote less screen time and more active fun!
The Live 5-2-1-0 Sleep Resource provides evidence for how following the 5-2-1-0 guidelines can support good sleep habits in children, and in turn, how sufficient restful sleep helps kids to make healthy choices during the day.
You can download and print the Sleep Resource below. Share it with parents, caregivers, and other service providers to promote a healthy sleep routine in children.
Leaders in the Stó:lō Service Agency, along with their partners in the Chilliwack Division of Family Practice’s Healthy Kids Initiative, adapted the Live 5-2-1-0 message to create the Live 5-2-1-0 Medicine Wheel. This resource shares the Live 5-2-1-0 message using the directions, terms, and colours from a lesson plan of the Ojibway tribe, and incorporates suggestions for healthy daily behaviours. The Live 5-2-1-0 Medicine Wheel can be used with children as a daily goal tracker to help them see how much healthy balance they have in their choices.
The Live 5-2-1-0 Medicine Wheel is generously shared by the Stó:lō Service Agency for use by other communities. If your community is interested in adapting their own version of the Live 5-2-1-0 Medicine Wheel to meet their own unique cultural needs, please contact SCOPE.
