Resources For Health Care Providers & Settings
Health care professionals directly impact children’s health. Every interaction is an opportunity to address and reinforce the importance of healthy lifestyle choices, good nutrition, and physical activity. Kids and parents take what health professionals say seriously!
Health professionals have a unique opportunity to emphasize the links between lifestyle choices and health to kids and families. Prevention of weight-related disease such as type 2 diabetes needs to begin early, and can be guided by the evidence-based, easy-to-remember Live 5-2-1-0 message.
SCOPE’s partner communities, in conjunction with local health partners, have developed a number of ready-made tools that can be adapted and adopted for use by health professionals to support the Live 5-2-1-0 message!
In partnership with SCOPE and Fraser Health, one of the very first initiatives implemented by our partner Healthy Abbotsford was the Public Health Nurse Live 5-2-1-0 Script for childhood immunization visits.
During Healthy Abbotsford’s process of community needs assessment and action planning, stakeholders identified a great opportunity to consistently educate new parents about the benefits of helping their children make healthy choices. In Abbotsford, all new parents see a Public Health Nurse when they take their children for regular scheduled childhood immunizations. Because this service is centrally coordinated in Abbotsford through their Public Health unit, it was possible to roll this initiative out across all childhood immunization clinics, which meant that the message would be reaching all new parents in Abbotsford.
In order to do this, Healthy Abbotsford and the Public Health unit of Fraser Health jointly developed a Live 5-2-1-0 Script for Public Health Nurses to use, which gave them an easy way to deliver consistent, evidence-based information. Partners at Public Health held trainings for the nurses, and provided copies of the script. The Live 5-2-1-0 Script introduces the Live 5-2-1-0 message to parents, explains the reasons why each of the guidelines is important for children in an easy-to-understand way, and offers some information about how to achieve these guidelines.
The Live 5-2-1-0 Public Health Nurse Script that was developed is available for download below, and can be adapted to add your community’s logo before use.
Lifestyle choices can have a big impact on children’s health, and talking about healthy choices with families is important. Parents and kids take what doctors and other health professionals say seriously, and giving them a prescription for healthier food, less screen time, more physical activity, or fewer sugary drinks might help a family to really try to make those changes.
We’ve developed a Live 5-2-1-0 Prescription Pad that you can use when talking to families about healthy choices, and that will help you to ‘prescribe’ part or all of the Live 5-2-1-0 guidelines to help families make healthier choices.
The Live 5-2-1-0 Prescription Pad can be downloaded below, and can be adapted to include your and your practice’s information and logos.
SCOPE, in partnership with the Chilliwack Division of Family Practice, has developed and piloted a Live 5-2-1-0 Family Physician Toolkit which aims to support Family Physicians in making BMI assessment and healthy living discussions part of regular family practice visits with children and youth. The Toolkit is based on existing evidence-based best practice, and provides information and resources to support family physicians in accurately measuring and classifying BMI percentiles in pediatric patients, assessment of healthy and unhealthy lifestyle choices, respectful discussions around healthy behaviours, motivational interviewing to support effective behaviour change, and other resources available locally to support families in making healthy choices.
Based on the results of the initial pilot phase in Chilliwack, adaptations were made to the Toolkit and a second pilot was completed with the East Kootenay Division of Family Practice in Kimberley. Currently, we have two key elements of the Live 5-2-1-0 Family Physician Toolkit available for download and use: the Assessment and Management Flow Chart, which is based on the ‘Pathway for the Identification, Assessment and Management of Overweight and Obese Children & Youth’ developed by Child Health BC in the Consensus Meeting on Weight Management for Children and Youth Final Report (March 2013); and the Live 5-2-1-0 Healthy Habits Questionnaires, which are a supporting tool for use with the Assessment and Management Flow Chart. These resources can be downloaded below.

The Live 5-2-1-0 Screen Time Fact Sheet provides the background for the guideline of no more than two hours of screen time a day, including its implications for children’s health and well-being.
The Screen Time Fact Sheet can be downloaded and printed below. Share it with your patients and their families to help discuss a balanced approach to the use of screens for children.
The Live 5-2-1-0 Sugar Drink Fact Sheet provides background for the guideline of zero sugar-sweetened beverages a day, including its implications for children’s health and well-being.
You can download and print the Sugary Drink Fact Sheet below and discuss with your kids why it’s important to limit the number of sugary beverages they are consuming, and to switch to the healthiest thirst quencher: water!
The Live 5-2-1-0 Sleep Resource provides evidence for how following the 5-2-1-0 guidelines can support good sleep habits in children, and in turn, how sufficient restful sleep helps kids to make healthy choices during the day.
You can download and print the Sleep Resource below. Share it with parents, caregivers, and other service providers to promote a healthy sleep routine in children.
